What should I include in the 6-session progress review?
- Please provide a brief clinical summary of the member’s progress so far, including:
- Whether they are showing signs of Reliable Improvement or Recovery
- Key goals achieved and areas still being worked on
- Why additional sessions are clinically appropriate (if needed)

Any risk or safeguarding concerns to be aware of
This helps ensure care is timely, measured, and aligned with both clinical best practice and AXA expectations.
Please mention the exact number of extra sessions that need to be added.
What if my member wants to use all 10 sessions up front?
You can still extend up to 10 sessions, but we encourage a reflective conversation with the member first.
Talk through:
Their progress toward therapy goals
Whether now is the best time to use all 10 sessions
The possibility of spacing or pausing sessions, if clinically appropriate, to consolidate progress or save remaining sessions for later use (depending on their policy)
Your clinical judgement as the treating clinician is always trusted and valued.
Where do I find the new AXA extension form?
The updated form is available in the Therapist Shared Resources folder HERE.
Please complete and email it as instructed. AXA aims to review requests within five working days.
Do I need to pause therapy while waiting for AXA to authorise sessions beyond 10?
Yes - therapy should be paused until you’ve received written approval from AXA for sessions beyond 10.
Sessions delivered without AXA approval may not be funded and therefore may not be paid.
Will this process apply to all AXA referrals?
Yes - these updates apply to all new and ongoing AXA referrals after 1st August 2025. All AXA trackers will now default to 6 sessions, with prompts built into the platform to support the new process.
